Job Description

A health services organization in Ohio is currently seeking a new Licensed Independent Social Worker (LISW), Licensed Professional Clinical Counselor (LPCC), and/or Licensed Professional Counselor (LPC) to join their team as a Social Work. In this role, the Social Worker (LISW / LPCC / LPC) will be responsible for providing essential therapeutic services within a supportive team environment with a strong dedication to community health and patient advocacy.

• Shift: Days
• Schedule: Full-time
• Hours: Based on clinic operating hours
• Setting: Federally Qualified Health Center (FQHC) / Primary Care
• Misc. Details: Opportunity for professional growth and to contribute to a team-based care model

Responsibilities:

• Providing therapeutic services to patients and families
• Collaborating with a team and external partners for a holistic care approach
• Advocating for patient needs to ensure positive health outcomes
• Performing other duties, as needed

Qualifications:

• 1+ year of experience in an Outpatient setting
• Graduate Degree in Social Work or related field
• Current OH Licensed Independent Social Worker (LISW), Licensed Professional Clinical Counselor (LPCC), and/or Licensed Professional Counselor (LPC)
• Ability to work collaboratively in a team-based care model
• Strong organizational skills for managing a diverse caseload
• Flexibility to cover for team members and participate in various operational roles
• Commitment to continuous professional development and learning

Desired Skills:

• Experience working within a Federally Qualified Health Center (FQHC)
• Experience with the SBIRT program
